GENERAL DESCRIPTION
Under the supervision of the CSD Director, Performs routine (journey-level) case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

ORGANIZATIONAL RELATIONSHIPS
Immediate Supervisor: CSD Director
Employees Supervised: None
Other: Communicates with clients, families, employers, service providers, community partnering agencies and the general public.

EXAMPLES OF WORK

– Interview potential clients by telephone, office visit and/or off-site visit to gather information to assess service needs and determine eligibility for CSD programs.
– Assists in identifying areas that are barriers to services for clients.
– Develops and implements service plans with clients and monitors client progress and tracks accomplishments.
– Collects program data in the CAP60 database and utilizes the database to case manage and provide services to CSBG clients.
– Coordinates with vendors to provide program services to eligible clients. Authorizes service delivery and coordinates service provider activities.
– Gathers documentation, compiles, and submits payable requests on behalf of client.
– Conducts reassessments and satisfaction surveys in accordance with program standards.
– Promotes community partnerships by promoting CSD-Social Services Coalition (CSD-SSC).
– Assists in the areas of Results Oriented Management and Accountability (ROMA), Organizational Standards, and National Performance Indicators.
– Assists in the establishment of positive partnerships with area resource providers and refers clients for assistance as appropriate.
– Attends training events, conferences, seminars, and meetings as approved by the Director.
– May represent the agency for community events and functions as approved by the CSD Director.
– Performs related duties as assigned.

GENERAL QUALIFICATION GUIDELINES
Education and Experience
Experience in case management or social services work. Graduation from an accredited four-year college or university with major course work in social work or a related field is generally preferred. Experience and education may be substituted for one another.

Knowledge, Skills and Abilities

– Experience examining household expenses and fiscal data to determine eligibility.
– Knowledge of community resources, of case management principles, objectives, standards, and program policies and procedures.
– Skilled in the use of office equipment, computers, and associated software applications. Microsoft Office computer skills (Word, Excel, Outlook).
– Ability to communicate effectively using correct grammar in written and oral forms.
– Able to utilize problem solving skills in all situations including crisis situations.
– Up to 30% of travel is required for client/vendor interaction and service delivery.
– In an office setting, not a remote telework position.

CERTIFICATES AND LICENSES REQUIRED
Valid Texas driver’s license and reliable personal transportation
